ePAC offers the following benefits to its employees. Most benefits are available to our full-time regular employees after the initial 90-day trial period which begins on the employees first day worked.

These benefit programs help our employees stay healthy, meet their financial goals, protect their incomes and their families, and balance the demands of work and personal life. The following list provides a brief overview of the available programs.

Medical
* Blue Shield PPO Plan - our PPO Plan offers employees and eligible dependents the option of basic coverage ($250 deductible for the employee and $500 deductible for family coverage). Some office visits and consultations are not subject to the calendar-year deductible but have a $15 co-pay on services from preferred providers (refer to coverage matrix). The PPO Plan gives employees freedom of their choice of plans but offers employees a lower cost if they choose an In-Network provider versus using an Out -Of-Network provider. Employees can choose their provider each time medical services are required.

* Access+ HMO Plan - the Blue Shield HMO offers employees and eligible dependents health care coverage as provided by the Blue Shield Network of Medical Providers. All medical care is managed by a primary care physician. In this plan there is no deductible and most services are paid at 100% after a $10.00 co-pay (with the exception of a few services listed on the coverage matrix). Lifetime Coverage-Unlimited.

* Kaiser HMO - In most cases office and hospital visits at Kaiser facilities are covered at 100% after a $10.00 co-payment.

Dental
* First Dental Health - Employees can choose to receive dental care through First Dental Health, but must pay the premium in full. The package includes a $25.00 deductible per individual ($75.00 family limit) for Preventative and $50.00 deductible per individual ($150.00 family limit) for major dental services. Preventative routine periodic examinations- 100% (once in any six-month period for routine services) not subject to deductible. Major Dental periodic examinations - 85% after deductible for a network provider, 70% after deductible for a non-network provider. There is an annual maximum of $1,500 per person per calendar year.

Vision
* Blue Shield - Blue Shield of California health plan members are eligible for a 20% discount with Eye Care Network Providers (ECN) on routine eye examinations, lenses, frames, contact lenses, photochromatic lenses, tints and coatings. To receive the discount all employees have to do is present their Blue Shield identification card when visiting a participating provider.

* Kaiser - Eye exams to provide a prescription for eyeglasses is $10.00 per visit. Optical coverage includes regular eye glass lenses every 24 months at no charge. Employees have a $80.00 allowance for eyeglass frames every 24 months and a $124.00 allowance for cosmetic contact lenses every 24 months in lieu of eyeglasses.

401K
* Employees may contribute up to 15% of their compensation on a pre-tax basis to our 401(k) plan. New hires can enroll in our 401(k) plan after 90 days of employment with ePAC.
